To help make your first visit to the Calm Spirit Wellness Clinic enjoyable, we request that you download and fill out the following forms and bring them with you.



When you come to your appointment, please wear comfortable, loose-fitting clothing.

Payment Options

We accept cash and check.  Also, in the state of Washington, many insurance plans cover acupuncture.  Contact your medical insurance provider to get pre-qualified.
